Mailing Address vs. Physical Address
The annual report must provide the physical address for all persons listed on the form. A physical address must contain a street or rural route, city, state and zip code.
If you provide a mailing address in conjunction with a physical address (111 Sewall Street, PO Box ABC, Augusta, ME 04330), the online form is designed to move the entire address to the mailing address field.
